Job summary

East Liberty Family Health Care Center in Pittsburgh is seeking a dedicated Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to diagnose, treat, and manage acute and chronic conditions in an outpatient setting. You’ll collaborate with physicians and support staff to deliver preventative, patient‑centered care in a mission‑driven community clinic.

Qualified candidates hold PA licensure in PA with national certification and 2+ years of clinical experience; bilingual abilities

Qualifications

• Active NP or PA licensure in Pennsylvania
• National certification (e.g., FNP, AGNP, PA-C)
• Minimum 2 years clinical experience preferred
• Experience in primary care, pediatrics, women's health, or internal medicine a plus
• Bilingual candidates encouraged to apply

Responsibilities

• Perform patient assessments, physical exams and health histories
• Diagnose and manage acute and chronic illnesses
• Prescribe medications and order diagnostic tests
• Provide patient education and counseling
• Collaborate with interdisciplinary teams to coordinate care
• Maintain accurate EMR documentation
• Participate in quality improvement and training initiatives
