Job summary

Generations Hospice, an affiliate of BrightSpring Health Services, is seeking a Nurse Practitioner to provide expert hospice care across Fond du Lac, Beaver Dam, Oshkosh and nearby areas.

You will perform patient assessments, symptom management, and care planning while ensuring alignment with patient goals. Travel within the territory is required; a fulfilling environment and interdisciplinary teamwork support your professional impact.

Qualifications

• Graduate of an accredited School of Nursing and Nurse Practitioner program.
• Master's degree with a minimum of one year NP experience.
• Current nursing license in the state of practice.
• Nurse Practitioner Certification in Gerontology, Adult, or Family (preferred).

Responsibilities

• Provide compassionate medical care and symptom management.
• Perform assessments, order/interpret diagnostic tests, and develop care plans.
• Prescribe medications, manage treatments, and coordinate hospice services.
• Conduct and document face-to-face visits for eligibility and recertification.
• Collaborate with physicians and IDT team to align care with patient goals.

Coverage area: Travel required throughout the territory for visits.

You will:

• Provide compassionate medical care and symptom management

• Maintain accurate clinical documentation and ensure compliance with regulations

• Serve as a clinical resource and liaison to community partners

Qualifications

• Graduate of an accredited School of Nursing and Nurse Practitioner program
• Master's degree with a minimum of one year NP experience
• One year of NP experience or one year of RN hospice experience
• Minimum of one (1) year hospice or palliative care experience preferred
• Current nursing license in the state of practice
• Current DEA license (if applicable to state regulations)
• Current CPR certification
• Certification from an approved advanced practice nursing certifying body, as required by the state
• Nurse Practitioner Certification in Gerontology, Adult, or Family (preferred)
• Excellent observation, verbal, and written communication skills
• Strong problem-solving and mathematical skills
• Proficient in nursing skills as outlined in the competency checklist
• Demonstrated ability to counsel and communicate effectively with patients and families regarding end-of-life issues
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
