Job summary

EPIC Healthcare is seeking a compassionate Nurse Practitioner (NP) to provide primary and ongoing care to residents in a skilled nursing facility in New Jersey. The NP will collaborate with physicians, nurses, and the interdisciplinary team to ensure high-quality medical care.

Responsibilities include conducting patient assessments, managing chronic conditions, coordinating with families, and ensuring accurate EMR documentation.

Qualifications

• Active New Jersey Nurse Practitioner license.
• Board Certified Nurse Practitioner (FNP, AGNP, or GNP) preferred.
• Experience in skilled nursing facility, long-term care, or geriatrics preferred.
• Strong clinical decision-making and communication skills.
• Ability to work collaboratively with nursing home staff.

• Respond to changes in residents' conditions and provide timely care.
• Order and interpret diagnostic tests and labs.

• Participate in care plan meetings and regulatory compliance.
