Altea Healthcare is seeking a Certified Nurse Practitioner (NP) for post-acute care in Oak Creek, Wisconsin. You will diagnose, treat, and guide patients to better health in a skilled nursing facility, with a focus on geriatrics and primary care.

Qualifications

• NP license required or eligibility to obtain.
• Proven ability to work effectively in a post-acute care setting.
• Strong communication and collaboration with multidisciplinary teams.

Responsibilities

• Diagnose, treat, and guide patients in a skilled nursing facility.
• Order and interpret diagnostic tests as appropriate.
• Prescribe medications and create treatment plans.
• Collaborate with physicians, nurses, and facility staff.
• Educate patients and families on health conditions and preventive care.
