Qualifications

• Two (2) or more years of Nurse Practitioner (NP) experience.
• Experience in Family Practice, Adult, Skilled Nursing, Long-Term Care, Geriatrics, Home-Based Primary Care, or Acute Care.
• Experience with PointClickCare or other Long-Term Care EHR systems.
• Knowledge of CMS Skilled Nursing Facility regulations.
• Familiarity with value-based care models, ACOs, I-SNPs, and geriatric prescribing principles.

Responsibilities

• Conduct medical exams, take medical histories, diagnose and treat common illnesses and injuries.
• Order and interpret laboratory tests and imaging studies as needed.
• Develop and implement individualized treatment plans, considering patient needs and health status.
• Prescribe medications and therapies as needed, and monitor patient response to treatment.
• Educate patients and families about healthy habits, disease prevention, and wellness strategies.
• Stay current with changes in medical policies, procedures, and regulations.
