Job summary

A Place At Home - Fairfield South is seeking a Non-Medical Post-Hospital Recovery Support caregiver to assist clients transitioning home after hospitalization, surgery, or medical treatment. You will help maintain a safe recovery environment and support independence under the care plan.

Responsibilities include personal care, mobility assistance, medication reminders (non-medical), meal prep, light housekeeping, and documenting care provided for team continuity.

Qualifications

• CNA, HHA, or equivalent caregiving experience preferred.
• Current certifications in First Aid, CPR, and TB test (or willingness to obtain).
• Successful completion of background check and drug screening, with random checks thereafter.
• Physically able to perform duties, including safe lifting and mobility assistance.

• Provide personal care assistance (bathing, dressing, grooming, toileting, mobility support) as directed in the care plan.
• Assist with safe transfers, walking, and repositioning to prevent falls and promote healing.
• Monitor and report changes in the client’s physical or mental condition to the supervisor.
• Support medication reminders (non-medical, per care plan).
• Prepare light meals and snacks according to dietary needs and assist with feeding if necessary.
• Help with light housekeeping, laundry, and organization to maintain a clean, safe environment.
• Provide companionship and reassurance during the recovery process.
• Document care provided and communicate with the care team to ensure continuity.
• Respect and maintain client dignity, privacy, and confidentiality at all times.

Essential Functions
