Emplify Health/Gundersen - Network is seeking a physician to help grow a dynamic Heart Failure program in La Crosse, Wisconsin. You will provide comprehensive care for patients across the HF spectrum, leveraging multimodality imaging and collaborating with a wide range of specialists to optimize outcomes.

The role supports growth of a young program with strong institutional support and a referral network for advanced therapies such as LVAD and transplant evaluation.

Formación

• Board-certified or eligible in internal medicine/cardiology and completed accredited graduate medical training.
• Ability to work within a multi-disciplinary team and manage heart failure patients across care continuum.
• Familiarity with advanced cardiovascular imaging and heart failure therapies is desirable.

Responsabilidades

• Grow the regional Heart Failure program and manage patients with acute and chronic HF.
• Provide comprehensive HF care across the disease continuum and reduce hospitalizations.
• Collaborate with cardiologists, EPs, PCPs, and teams to optimize patient outcomes.
• Utilize advanced imaging for diagnosis, risk stratification, and management decisions.
• Coordinate referrals for LVAD, transplant evaluation, or other advanced therapies.
• Expand community outreach and referrals to improve access to HF care regionally.
