Sweetwater Care seeks an experienced LVN nurse manager to join our team in California. The role focuses on delivering high-quality nursing care within the approved scope of practice, aligning with physician orders and facility policies.

You will participate in quality improvement activities, educate patients and families, and collaborate with other departments to ensure smooth patient transitions and timely medication administration. Previous LTC and PointClickCare experience are valued assets.

Qualifikationen

• Currently licensed as LVN in California.
• One-year nursing management experience preferred, not required.
• Long term care experience preferred.
• PointClickCare charting experience required or preferred.

Aufgaben

• Provide nursing care within scope of practice according to standards and physician orders.
• Participate in and support the Quality Improvement Program.
• Identify, place, and monitor patients in appropriate programs using the Quality Life Manual.
• Communicate patient changes to other departments (room changes, LOA, admits, discharge).
• Educate patients and families.
• Administer medications timely and according to facility policy.
• Administer treatments as ordered and per policy.
• Receive, transcribe and carry out physician orders as allowed by the Nurse Practice Act.
• Make rounds with physicians as needed.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
• One-year prior nursing management experience preferred, not required.
