Job summary

Maybelle Center in Portland, OR is seeking a licensed Practical Nurse (LPN) to monitor resident health, administer medications, assist with daily care, and maintain accurate medical records. You will support the healthcare team, implement individualized Service Plans, and ensure compliance with Oregon rules.

The NOC shift (10:00 pm–6:30 am) serves as Manager on Duty, coordinating care and reporting concerns to on-call staff.

Qualifications

• Current LPN license valid in Oregon.
• 1–2 years of clinical experience.
• Experience in an assisted living facility preferred.
• Ability to work with a multidisciplinary healthcare team.

Responsibilities

• Monitor residents’ health and administer medications and treatments.
• Assist with ADLs, environmental orientation, and medications.
• Document care in EMR and update care plans as needed.
• Coordinate with pharmacies, physicians, and home health agencies.
• Report changes in condition promptly and maintain chart accuracy.
