Job summary

LiveWell Homecare Agency, LLC in Wichita, KS is seeking an experienced registered nurse for full-time night shifts (7:00 pm–7:00 am, 3 nights per week) to lead holistic patient care and coordinate personalized care plans.

You will monitor clinical conditions, advocate for patients, and coordinate discharge goals to ensure smooth transitions home. KS RN license and BLS within 30 days are required; sign-on bonus up to $10,000.

Qualifications

• Valid RN license from the Kansas Board of Nursing.
• BLS certification within 30 days of hire.
• Diploma from an accredited nursing program.

Responsibilities

• Lead holistic patient care by executing personalized care plans and monitoring clinical conditions.
• Serve as the primary care coordinator, facilitating communication among multidisciplinary teams.
• Advocate for patients by translating physician orders into bedside actions while maintaining high clinical standards.
• Support patient transition to home by coordinating discharge needs and ensuring families feel confident and supported.
