Job summary

Singing River Health System is seeking a Patient Care Coordinator (PCC) to assist patients in attaining, maintaining, and restoring health. The PCC will manage a designated clinical area, organize care, and make/alter assignments based on patient needs while leading performance improvement projects.

Education: NLN School of Nursing graduate; BSN preferred. Licensure: Mississippi RN license in good standing.

Qualifications

• Graduate from NLN School of Nursing required.
• Bachelor of Science in Nursing (BSN) preferred.
• Currently licensed to practice as a Registered Nurse in the State of Mississippi.
• All RNs are required to complete and maintain BCLS certification upon hire and/or transfer.
• Specialty areas require ACLS, PALS, TNCC, NRP certifications as listed.
• RNs have 12 months, from the date they begin working in the specialty area, to acquire the appropriate certification and 18 months to acquire TNCC certification for the Emergency Department.
• Must have current TB certification applicable to position and scope of practice.
• Must have de‑escalation training completed by the end of position orientation (90 days).
• Must have appropriate level of de‑escalation training.
• Two years’ experience as a registered nurse preferred.

Responsibilities

• The PCC assists patients attain, maintain, and restore health when possible.
• Manages a designated clinical area with key responsibilities.
• Organizes and facilitates care by making/altering assignments based on patient needs.
• Leads performance improvement projects and collaborates with staff to improve outcomes.
• Adheres to hospital procedures, policies, and governing guidance.
