Banner North Colorado Medical Center (Greeley Loveland) seeks an experienced RN for a full-time night shift in the Float-Hosp unit. Responsibilities

• Assesses patient physical, psychological, social, spiritual, educational, developmental and discharge planning needs.
• Formulates a plan of care including discharge planning and goals, in collaboration with the patient/family.
• Implements the plan of care through direct patient care, coordination and supervision of the health care team.
• Evaluates patient progress toward goals and documents outcomes in the patient record.
• Promotes continuity of care by communicating status and plans to other caregivers.
