Job summary

Trinity Health MI in Grand Rapids, MI seeks a licensed Registered Nurse for a part-time, 12-hour night shift. The On-Site Virtual RN will join the Nursing Care Team to plan, coordinate and deliver patient-centered care via a digital platform.

The role requires BSN, active RN license in Michigan, and at least five years of RN experience with adult condition management. EMR experience and a valid driver's license are also needed.

Qualifications

• BSN from an accredited school of nursing.
• Active RN license in applicable state(s).
• Valid driver's license where required by assignment.
• Specialty credentialing or advanced clinical degree as per specialty.
• Minimum five years of RN experience including adult condition management.
• Experience with Electronic Medical Records (EMR).

Responsibilities

• Collaborate as part of the Nursing Care Team through a digital platform supporting daily management of care.
• Assess, plan, implement, coordinate, monitor and evaluate the patient’s care from admission to discharge.
• Utilize clinical knowledge, critical thinking and case-management principles to coordinate and support the care plan.
• Ensure a seamless, effective transition of care across the continuum.
• Document accurately in the patient record and monitor department quality metrics.
• Serve as mentor, clinician, educator, consultant/collaborator and navigator.
