Qualifications

• Must be a Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a current MA license.
• Ability to work night shift, including weekends and holidays.
• Provide direct patient care and follow individualized care plans.

Responsibilities

• Provide direct patient care to residents and patients on a night shift schedule, including administering medications, wound care, and other treatments as needed
• Assess patient conditions, develop and implement individualized care plans, and collaborate with other healthcare professionals to ensure optimal patient outcomes
• Maintain accurate and detailed records of patient care, as well as perform comprehensive chart reviews and quality assessments
• Develop and implement education programs for patients, families, and staff, focusing on disease prevention, symptom management, and wellness strategies
• Participate in quality improvement initiatives and performance improvement projects to enhance patient care and outcomes
• Communicate effectively with patients, families, and healthcare team members to ensure seamless transitions and coordination of care
