Job summary

Elm-Terrace-Gardens in Lansdale, PA seeks a compassionate LPN Supervisor to oversee Med Techs and CNAs in a personal care community. The role focuses on high-quality resident care, safety, and coordination with the healthcare team.

Qualifications

• Valid and current LPN license in PA.
• 2+ years supervisory experience in personal care or skilled nursing.
• Strong interpersonal and communication skills.
• Compassionate, patient and respectful with residents.
• Ability to follow detailed instructions and safety protocols.
• Good organizational skills and attention to detail.
• Ability to lead a team and make emergent decisions.
• Physical ability to assist with mobility and caregiving tasks.

Responsibilities

• Supervise the care team (MED TECHs, CNAs, Direct Care Workers) to ensure high-quality care.
• Handle Admissions, Discharges, and ongoing assessments of residents.
• Monitor health status and vital signs; document changes.
• Administer medications and treatments per providers.
• Perform cart audits, RASPs, and DME documentation.
• Support residents with activities of daily living; promote independence.
• Maintain thorough documentation of care and resident progress.
• Collaborate with physicians and therapists to implement care plans.
• Ensure a safe, clean, and comfortable environment at all times.
