Meadow Falls of Madison in Madison, OH is seeking a compassionate Licensed Practical Nurse (LPN) for the night shift to provide high-quality care in a resort-style senior living community offering Assisted Living, Memory Care, Adult Day, and Respite Care.

Responsibilities include administering medications, wound care, daily living support, and collaborating with the care team to develop and implement care plans, while maintaining detailed care records.

Qualifications

• Active LPN license in Ohio.
• High school diploma or equivalent.
• Experience in long-term care or gerontological nursing preferred.

Responsibilities

• Provide high-quality, individualized care to residents, including administering medication, wound care, and assisting with daily living activities.
• Develop and implement individualized care plans with the interdisciplinary care team.
• Monitor and report changes in resident condition to nursing staff and healthcare providers.
• Participate in resident assessments, care planning, and coordination of services with other healthcare professionals.
• Maintain accurate and detailed records of resident care, including medication administration and treatment plans.
• Provide emotional support and companionship to residents and their families.
• Collaborate with the interdisciplinary care team to ensure seamless transitions and care coordination.
