Job summary

Responsibilities include continuing assessment of residents, ensuring the plan of care is followed by the team, and communicating with families and third-party providers to address health concerns.

Qualifications

• Must have compassion and a passion for helping others.
• Ability to multitask in a fast-paced environment.
• Excellent communication and interpersonal skills are required.
• Knowledge of HIPAA and confidentiality standards.
• Experience with PCC or similar EMAR system preferred.
• Licensed by the Louisiana Board of Practical Examiners in good standing.

Responsibilities

• Continued assessment of residents to promptly address health issues.
• Work with the multidisciplinary team to ensure the plan of care is followed and updated.
• Communicate with third party providers and family regarding health and wellbeing.
