Job summary

Trustwell Living at Mansfield Place in Mansfield, OH is hiring full-time LPNs and on-call staff to provide direct resident care according to individualized care plans. You will monitor residents, document changes, and support the health services team to ensure quality care around the clock.

The successful candidate will hold a valid LPN license, have at least one year of long-term care or assisted living experience, and be prepared to handle medication administration and mandatory reporting

Qualifications

• LPN license required and in good standing.
• Minimum of 1 year of LPN experience in assisted living or long-term care.
• Experience administering meds per orders and policies.
• Knowledge of state resident care regulations and survey readiness.
• Experience as a mandatory reporter and ability to intervene, report, and investigate abuse or theft.
• Valid driver’s license.

Responsibilities

• Provide direct resident care and supervision per care plan.
• Monitor changes in resident condition and document properly.
• Communicate with Health Services Director and supervise care team.
• Ensure compliance with federal/state laws and community policies.
