Job summary

GREEN ACRES is seeking a licensed LPN/RN for full-time night shift and weekend coverage. You will coordinate with the healthcare team to plan and deliver patient care, administer medications, and monitor patients’ conditions.

Qualifications

• Current LPN/RN license in the state.
• At least one year of LPN or RN experience in a long-term care setting is preferred.

Responsibilities

• Consults and coordinates with healthcare team members to assess, plan, implement and evaluate patient care plans.
• Prepares and administers medications (oral, subcutaneous, IV) and records prescribed medications.
• Records patients' medical information and vital signs.
• Monitors and adjusts equipment used on patients and records readings.
• Initiates corrective action when patient shows adverse symptoms.
• Provides bedside care for a wide variety of medical patients.
