Job summary

Lifepoint Health® operates Rehabilitation Hospital of Wisconsin, seeking a Licensed Practical Nurse (LPN) for full-time night shifts. You will provide direct and indirect patient care, develop personalized care plans, and support discharge processes within a collaborative care team.

The role requires an Associate's Degree, Wisconsin LPN licensure (or compact state), BLS certification within 30 days, and a preferred minimum of one year of experience.

Qualifications

• AHA Basic Life Support (BLS) within 30 days of hire.
• Requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
• One year previous experience preferred.
• Active Wisconsin or compact state LPN license required. If relocating, licensure must be updated to new PSOR before employment begins.
• IV Certified - Highly Preferred or must obtain within 6 months of start.
• Associate's Degree required.

Responsibilities

• Deliver direct and indirect patient care using the nursing process-Assessment, Planning, Intervention, and Evaluation
• Collaborate closely with patients, families, and our interdisciplinary care team to create personalized care plans
• Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues
• Play a key role in the discharge planning process to support safe, successful transitions
