Job summary

Qualifications

• Graduate of an approved school for practical or vocational nursing is required.
• Must be currently licensed by the State Board of Nursing.
• Experience in rehabilitative or geriatric nursing preferred.
• English proficiency for patient safety and communication.

Responsibilities

• Collects, reports and documents objective and subjective data.
• Observes changes in condition and reports them to the RN.
• Care planning: contributes to goals and develops interventions.
• Implements the plan of care and evaluates effectiveness.
• Provides direct patient care and administers medications per orders.
• Documents thoroughly and communicates with the RN/physician.
• Assists in discharge planning and prepares patient/family for discharge.
• Monitors and supervises unlicensed staff and delegates tasks.
• Uses PCC according to policy and performs drug counts.
• Collaborates with other departments to ensure safe, patient-centered care.
• Maintains confidentiality and promotes a culture of safety.
