Responsibilities

• Delivers safe and appropriate care to patients according to nursing skills inventory and patient needs.
• Coordinates planning for nursing care with an interdisciplinary team.
• Collaborates with healthcare team to achieve positive patient outcomes based on standards of care.
• Coordinates treatment plans and develops discharge or transfer plans.
• Participates in program evaluation, process improvement, and patient-centered outcomes.
• Addresses diverse patient needs across the continuum of care, ensuring quality service.
• Adheres to regulatory standards and Memorial Hermann policies.
• Promotes professional growth, may mentor less experienced staff.
