Qualifications

• Associate degree in nursing is required or equivalent.
• BSN preferred depending on unit
• Strong critical thinking and communication skills required.

Responsibilities

• Conducts an initial assessment within 30 minutes of admission.
• Develops and updates outcome-based care plans coordinating with the care team.
• Documents discharge instructions and communicates with physicians, patients and families.
