Qualifications

• Graduation from ACPE accredited School of Pharmacy with a BS or PharmD.
• Current license to practice Pharmacy in the state of practice.

Responsibilities

• Prepares and dispenses medication orders per physician request according to policies.
• Reviews and interprets medication orders ensuring accurate entry to the patient profile.
• Compounds and dispenses pharmaceuticals, including sterile, chemotherapy, and PN products.
• Issues controlled substances to patient care areas and maintains records as required.
• Monitors drug therapy regimens for contraindications, interactions, and dosing.
• Provides clinical consultation and drug information to practitioners.
• Supervises and directs pharmacy support personnel.
