SSM Health Hospice House Baraboo in Baraboo, WI is seeking a part-time Medication Aide to assist with medication administration under a nurse's direction. The role requires CNA and Medication Aide Training, plus BLS certification, and involves night shifts with a weekend rotation.

You will uphold state and federal regulations while delivering compassionate resident care in a hospice setting. The position emphasizes accurate med distribution, careful documentation, reporting changes, and teamwork

Qualifications

• Must have Wisconsin CNA certification and Medication Aide Training.
• BLS HCP certification required.
• Experience in geriatric care preferred.

Responsibilities

• Maintain accurate medication inventory and perform checks prior to administration.
• Distribute medications per care plan and monitor for side effects.
• Provide care for subacute/chronically ill residents and assist with transfers.
• Respond to alarms, assist with mobility, and support meal service.
• Complete documentation for medications and treatments in a confidential manner.
• Communicate changes in resident condition to the licensed nurse promptly.
