Nexushm is seeking a Licensed Practical Nurse (LPN) to provide high-quality resident care on a full-time night shift. You will assess needs, implement care plans, and work with a multi-disciplinary team to ensure optimal outcomes.

Responsibilities include administering meds, monitoring progress, documenting findings, and educating patients and families on health conditions and aftercare. Illinois license and CPR are required, with LTAC experience preferred.

Qualifications

• Illinois nursing license in good standing.
• CPR certification.
• At least one year of professional experience in long-term care.
