Qualifications

• Doctor of Pharmacy or BS Pharmacy degree required.
• Hospital experience preferred.
• Pharmacy state licensure required within 60 days of start date.
• BLS or ACLS is required.
• PALS (for pediatric practice) is required.

Responsibilities

• Prepares and dispenses drug orders per physician request according to established policies, procedures, and protocols.
• Interprets drug orders (verbal and written) and transcribes/verifies into computerized patient medication record accurately.
• Compounds and dispenses pharmaceuticals including non-sterile and sterile products, chemotherapy and parenteral nutrition products.
• Issues controlled substances to patient care areas and maintains records as required by law and institutional policies.
• Provides ambulatory care pharmaceutical services, patient education, and participates in clinical initiatives as assigned.
• Maintains current pharmacist license and CE records; attends staff meetings.
