Qualifications

• Assesses patient needs and develops an individualized plan of care.
• Collaborates with medical staff and interdisciplinary team.
• Documents patient care in a manner meeting regulatory standards.
• Maintains regular attendance per policy and contributes to unit goals.
• Performs duties as assigned and supports patient and family needs.
• Provides care within Hospital Care at Home and in-home nursing programs.

Education
