Job summary

Responsibilities

• Complete nursing assessments and document findings.
• Develop and update the plan of care based on assessment.
• Collaborate with physicians and healthcare team on the plan.
• Provide, delegate, and coordinate care delivery.
• Initiate discharge planning and teach patients/families.
• Maintain policies/procedures and monitor patient responses.
