Qualifications

Responsibilities

• Assess physical, social, and psychological status and information from interdisciplinary team.
• Plan individualized care with patient, family, and team involvement.
• Implement care by directing the care team and coordinating resources.
• Evaluate patient response and update the plan of care as needed.
• Maintain professionalism, documentation, and compliance with licensure and regulations.
