Job summary

Requires accredited PT training (Bachelor/Master/Doctorate) and California license, BLS, and NPI, plus adherence to department policies and patient education responsibilities.

Qualifications

• Completion of an accredited PT training program with a Bachelors, Masters, or Doctorate degree in Physical Therapy.
• Licensed Physical Therapist in California at hire or eligible, with CA license within 4 months if required.
• Basic Life Support (AHA) at hire and National Provider Identifier at hire.
• Ability to work in a Labor/Management Partnership environment.

Responsibilities

• Evaluates patients and interprets findings to determine dysfunction and restore function.
• Establishes plan of care to achieve goals based on patient age and history.
• Administers modalities, therapy, and prescribed exercise programs.
• Documents services accurately per state and policy requirements.
• Coordinates discharge planning and participates in team rounds.
