Job summary

Hackensack Meridian Health is seeking a nurse navigator to coordinate patient care with surgeons and the health team, guiding patients through complex care pathways.

You will assess educational, financial, psychosocial, nutritional, and clinical needs, connect patients to resources, and facilitate access to appropriate clinical trials when eligible.

Qualifications

• RN with at least 5 years in a relevant clinical specialty.
• Active NJ State RN license.
• BLS/ACLS certification.
• Proficient in MS Office.
• Strong organizational and critical thinking skills.

Responsibilities

• Coordinate patient care with surgeons and providers and the healthcare team.
• Assess patient and family needs including educational, financial, psychosocial, nutritional and clinical.
• Identify eligibility for clinical trials and facilitate access to care.

The nurse navigator in collaboration with the surgeons and/or providers and the entire healthcare team navigates the care for patients including but not limited to: ongoing assessment of patient and family needs including educational, financial, psychosocial, nutritional, and clinical; connecting patients and families to available resources within the health system and in the community, assessing patients for eligibility for clinical trials and facilitates access to care.

Education, Knowledge, Skills and Abilities Required:

• Graduate of licensed school of nursing;

• RN with at least 5 years of clinical experience in a relevant clinical specialty.

• Proficient computer skills- proficiency in MS Office.

• Strong organizational, time management, critical thinking RN skills.

• Ability to work independently and exercise sound judgment in interactions with physicians, APNs, patients, and their families.

Education, Knowledge, Skills and Abilities Preferred:

• BSN

Licenses and Certifications Required:

• NJ State Professional Registered Nurse License.

• BLS AHA Basic Health Care Life Support HCP Certification.

Licenses and Certifications Preferred:

• National Certification.

Minimum rate of $113,609.60 Annually
