• BLS at hire.
• Flexible scheduling to complete program.

Responsibilities

• Implement the assessment plan with follow-up of the care plan.
• Assess physical, emotional, social, educational and functional needs.
• Interpret data to develop a nursing care plan with goals and discharge planning.
• Collaborate with interdisciplinary team on rehabilitation goals.
• Ensure medical records are complete, accurate and timely.
