Job summary

Human Touch Home Health in Blacksburg, VA is seeking a New Graduate Physical Therapist to join our home health team. You will evaluate patients in their homes, build individualized plans of care, and provide exercise and mobility training to support recovery.

New graduates are encouraged to apply. A Virginia license, reliable transportation, and strong communication skills are essential. We offer tuition reimbursement and comprehensive benefits through full-time employment.

Qualifications

• Graduate of an Accredited Physical Therapy Program.
• Active Virginia Physical Therapist License Required.
• New Graduates Encouraged to Apply.
• Valid Driver's License and reliable transportation.
• Strong communication and patient-care skills; willingness to learn in home health.
• Ability to work independently while knowing when to ask for support.

Responsibilities

• Complete physical therapy evaluations in patients' homes.
• Develop individualized plans of care.
• Provide therapeutic exercise, gait, balance, transfer, and mobility training.
• Assess home safety and fall risks.
• Educate patients and caregivers.
• Monitor patient progress.
• Coordinate care with nurses, therapists, physicians, and other team members.

• Participate in discharge planning.
