Job summary

Oak Street Health is offering a New Graduate Nurse Practitioner Fellowship in Bronx, NY, aligned with the University of Michigan School of Nursing. The 12-month, paid program prepares NPs for adult primary care within a value-based care model.

Fellows develop a patient panel, learn from preceptors, and engage in didactic content to build on graduate training. Upon completion, you may continue with Oak Street Health.

Qualifications

• Family, adult, or adult-gerontology primary care nurse practitioners who graduated or will graduate from their programs between December 2024 and May 2026.
• Must reside in or relocate to the fellowship location for at least two years.
• Have not yet worked as an NP.
• Minimum of one year experience as an adult RN in the last five years.
• Minimum GPA of 3.0 in NP program.
• Have passed, or will pass, NP board examinations by 6/6/2026.
• Active NP license by 8/3/2026.
• Passion for adult primary care and teamwork with older adults.
• Oaky values and collaboration with colleagues.

Responsibilities

• Build your own patient panel and deliver care in a value-based model.
• Learn from multiple preceptors and engage in didactic content.
• Commit to working at Oak Street Health for at least one year after the program.
