Qualifications

• Master’s Degree as a Nurse Practitioner or Physician Assistant (or graduated from an accredited PA program).
• NYS Licensed/Registered NP or PA.
• Current National Certification consistent to the clinical practice.
• Having a NPI number and Medicare enrollment with a Medicare Identification Number (or PTAN) or eligibility for Medicare Enrollment within 6 months of hire.
• CPR-BLS Certification required at the time of appointment.
• Must successfully pass pre-employment and credentialing clearances.

The Neuroscience Advanced Practice Provider will serve in a Per Diem role to primarily provide care to patients on the Neuro ICU service. The APP will provide inpatient coverage at Upstate Downtown Campus.

Key Responsibilities

• Provide expert level clinical care and coordination of care via a full range of services that will improve quality of patient care.
• Assess and evaluate patients, order appropriate laboratory testing and diagnostic studies, arrange for referrals, testing, consultation and therapeutic services.
• Maintain outstanding relationships with families, referring physicians and all members of the health care team.

Minimum Qualifications

• Master’s Degree as a Nurse Practitioner or Physician Assistant (or graduated from an accredited PA program).
• NYS Licensed/Registered NP or PA.
• Current National Certification consistent to the clinical practice.
• Having a NPI number and Medicare enrollment with a Medicare Identification Number (or PTAN) or eligibility for Medicare Enrollment within 6 months of hire.
