Qualifications

• Minimum 6 months of experience in long-term care or skilled nursing facility.
• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• DEA license.
• CPR BCLS/BLS certification.
• One year of experience in Neurology.
• Preferred experience documenting EMR.

Responsibilities

Location: South Central and Southeast CT
Schedule: Full-Time (5 days per week)

Job Summary

We seek a friendly and compassionate Neurology Nurse Practitioner/Physician Assistant to join our team. In this role you will provide comprehensive care to patients with neurological disorders including epilepsy, multiple sclerosis, Parkinson’s disease, and other conditions, making a difference in patients’ lives while working alongside a dedicated healthcare team.

Responsibilities

• Develop and implement individualized treatment plans, including medication management and non-pharmacologic interventions.
• Conduct regular follow-up visits to monitor patient progress, adjust treatment plans, and provide education and support.
• Collaborate with physicians and other healthcare professionals to coordinate care and ensure a multidisciplinary approach.
• Provide patient and family education on disease management, medications, and lifestyle modifications.
• Maintain accurate and timely documentation of patient encounters and treatment plans.
• Participate in quality improvement initiatives to enhance patient outcomes and optimize care delivery.
• Perform comprehensive assessments to accurately diagnose and treat patients with neurological conditions.

Requirements

• Minimum 6 months of experience in long‑term care or a skilled nursing facility.
• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• DEA license.
• CPR BCLS/BLS certification.
• Master’s degree in Nursing.
• Completed a degree from an accredited Nurse Practitioner program.
• One year of experience in Neurology.
• Preferred experience documenting EMR (Sigma, Visual, PCC, etc.) – training provided.
• Excellent organizational and time‑management skills.
• Able to lift a minimum of 20 pounds and stand, walk, or work on your feet for 6–8 hours per day.
