Job summary

CommonSpirit Health is seeking an Advanced Practice Provider in Lakewood, Colorado, to lead rapid assessment and nursing diagnoses in collaboration with MD partners. You will perform advanced practice procedures and apply expertise to ensure comprehensive patient care, following organizational mission and core values.

The role requires an RN with advanced assessment skills, diagnostic testing expertise, and collaboration with physicians, plus accountability for patient outcomes.

Qualifications

  • Master's Degree in Nursing as Nurse Practitioner from an accredited program.
  • RN license in good standing with required state APRN certification.
  • Board certification as APRN in applicable population foci.

Responsibilities

  • Assess patients' physical, psychosocial, cultural, and spiritual needs using observation, interview, and records.
  • Evaluate patient responses and intervene promptly to ensure optimal outcomes.
  • Collaborate with interdisciplinary team to develop holistic, individualized care plans.
  • Assess learning needs and document patient and family understanding.
  • Implement care plans and monitor outcomes per nursing standards and physician direction.
  • Maintain patient charts and records with thorough confidentiality.

Job Summary And Responsibilities

As our Advanced Practice Provider, you will assume responsibility for rapid assessment and nursing diagnosis, collaborating with MD partners to ensure safe patient diagnostic testing and an optimal plan of care.

Every day you will perform advanced practice procedures, utilize the nursing process to promote/restore optimal health and well-being, and apply your expertise to ensure comprehensive patient care, all while adhering to our organizational mission and core values.

To be successful in this role, you will be a Registered Nurse with advanced clinical assessment skills, expertise in diagnostic testing and care planning, with the ability to effectively collaborate with physicians while demonstrating personal accountability for patient outcomes.

  • Assesses patients' physical, psychosocial, cultural, and spiritual needs through observation, interview, reviews of records, and clinical data.
  • Evaluates patient's response and intervenes in a timely manner to ensure optimal patient outcome.
  • Collaborates with interdisciplinary team to develop and implement holistic, individualized plan of care.
  • Assesses patient/family learning style and appropriately teaches and documents understanding.
  • Implements patient plan of care and observes outcomes in accordance with nursing standards and physician direction.
  • Thorough and confidential maintenance of patient charts and records.

Required

Job Requirements

  • Master\'s Degree in Nursing as Nurse Practitioner, from a nationally accredited postgraduate certificate program. Must be a graduate from an Accredited School of Nursing
  • Registered Nurse Practitioner Licensure for; and listed on the state Advanced Practice Nurse (APN) Registry. Board certified as an Advanced Practice Registered Nurse, in applicable population foci and/or subspecialty
  • RN License; Current state nursing licenses or valid RN license from a participating state in the National Licensure Compact
  • Basic Life Support, may be required depending on practice setting
  • Advanced Cardiac Life Support, may be required depending on practice setting
  • RXN/RXNP Have Prescriptive Authority (RXN) or have Provisional Prescriptive Authority (RXNP), if applicable for role with associated articulated plan
  • NRP is for both hybrid work in person and telemedicine neonatology at SANHC, LUH, SFMC, MRMC (include in PB as needed), BLS not required for these departments

Preferred

  • One (1) year of experience in a clinical or advance practice setting
