Qualifications

• Education: Bachelors Degree, Nursing (Preferred) or Associates Degree, Nursing (Preferred) or Program Graduate, Nursing (Nursing Diploma is minimum education requirement, Associate’s or Bachelor’s in Nursing accepted in lieu of Nursing Diploma)

Responsibilities

• Assess patient clinical and psychosocial needs by applying standards of practice for the specialty or patient population.
• Demonstrate unit-specific competencies and equipment training.
• Partner with patients and families to develop and evaluate a plan of care and treatments.
• Collaborate with interdisciplinary team for care coordination.
• Facilitate safe patient transition to the next level of care.
• Document and evaluate patient education and discharge instructions.
