Qualifications

Responsibilities

• Interpret referrals and evaluate patients with speech, language, voice, cognitive and swallowing disorders.
• Establish a written plan of care and implement treatment; monitor outcomes and adjust as needed.
• Complete discharge planning including home programs and family education; coordinate with care team.
• Document per department policy; maintain productivity and billing records; ensure compliance.
• Promote patient safety and participate in quality initiatives; support ongoing professional growth.
