Job summary

Nexus Neurorecovery Center – Conroe is seeking an experienced RN Case Manager to coordinate care for patients with brain injuries, neurological conditions, and complex rehabilitation needs. This role emphasizes interdisciplinary collaboration, care planning, and transitions of care from admission through discharge.

Ideal candidates have 3–5 years in acute-care nursing, 2–3 years in case management or discharge planning, and an active Texas RN license.

Qualifications

• Graduate of an accredited School of Nursing.
• Associate Degree in Nursing required.
• BSN preferred.
• Active Texas RN license required.

Responsibilities

• Conduct comprehensive assessments of patients’ medical, psychosocial, emotional, and discharge needs.
• Develop and continually update individualized care plans based on patient needs and treatment goals.
• Coordinate care across physicians, nursing, rehabilitation, behavioral health, and other team members.
• Participate in multidisciplinary rounds and lead team conferences.
• Monitor progress and identify barriers impacting treatment, length of stay, or discharge.
