Job summary

SSM Health Rehabilitation Hospital - Neuro Transitional Center in O'Fallon, MO is seeking a Physical Therapist for a per diem role opening June 2025. The Neuro Transitional Center provides intensive rehabilitation for individuals with acquired brain injury or spinal cord injury who cannot safely return home yet.

Qualifications

• Current state licensure or eligibility to practice Physical Therapy.
• BLS certification (or ability to obtain within 90 days).
• Valid driver's license in good standing.
• One or more years of clinical experience in neuro rehabilitation or neuro transitional care.

Responsibilities

• Evaluates assigned patients and develops a plan of care with the transdisciplinary team.
• Delivers therapy across multiple environments and trains Neuro Rehab Coaches.
• Serves as primary therapist for assigned patients and maintains a structured weekly schedule.
• Completes medical records documentation and charge entry in compliance with policies.
• Participates in discharge planning and family education for safe community reintegration.
