Collage Home and Community Rehabilitation Partners in Oceanside, CA is seeking experienced Licensed Clinical Social Workers to provide patient-centered, in-home care for individuals with traumatic brain injuries, spinal cord injuries, and other neurological or complex orthopedic diagnoses.

This PRN role offers flexible assignments across Oceanside and surrounding communities, with interdisciplinary teams developing personalized treatment plans and supporting patients and families in the comfort

Qualifications

• Master’s degree in Social Work required and state licensure as an LCSW.
• Excellent planning, organization, writing, and time management skills are required.
• Ability to communicate with families, professionals, and co-workers is a must.

Responsabilités

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification
