Job summary

Qualifications

• Current RN licensure in Illinois or eligible.
• BLS certification required prior to start date.
• BSN degree preferred.
• Experience in a healthcare environment preferred.
• Electronic Medical Records and Microsoft applications basic proficiency.

Responsibilities

• Provide care coordination across the continuum.
• Assess and support physical and psychological needs of patients and families.
• Collaborate with providers and multidisciplinary care teams in a 24/7 hospital setting.
• Mentor assigned team members and maintain professional disposition.
