Job summary

Advocate Health Care is seeking an Advanced Practice Provider to assess, diagnose, and manage patient care within an interdisciplinary team. You will order and interpret diagnostics, plan treatments, and coordinate care while adhering to evidence-based practices and billing requirements.

You will provide health promotion, disease prevention, and education to patients and families, maintain precise EMR documentation, and perform procedures within your privileges to ensure timely, safe care

Qualifications

• Active RN and APRN license in state(s) of practice, AND board certification, AND DEA registration.
• Master's Degree in Nursing is required.
• 1–2 years of APP practice experience.

Responsibilities

• Assesses, diagnoses, and determines treatment and management plans for age, acuity and condition.
• Manages conditions based on evidence, cost effectiveness, and risk/benefit analysis.
• Provides health promotion, disease prevention, and education for patients and families.
• Works within an interdisciplinary team and seeks referrals as needed; escalates care when required.
• Maintains accurate EMR documentation and submits services per billing practices.
• Facilitates coordinated care and clear communication among healthcare team and agencies.
• Performs procedures per specialty privileges and scope of practice where applicable.
