Job summary

Dartmouth-Hitchcock is seeking a full-time Nephrology Advanced Practice Provider to join its Nephrology/Hypertension Clinic in Manchester, New Hampshire. This position involves providing outpatient direct patient care and coordinating care for patients at community hospitals, focusing on comprehensive nephrology services.

Qualifications

• Eligible for RN licensure in the State of NH (APRN only).

Job description

Dartmouth‑Hitchcock’s Nephrology/Hypertension Clinic is seeking a full‑time Nephrology Advanced Practice Provider to join our practice in Manchester, New Hampshire. This position is responsible for outpatient direct patient care and the coordination of care for patients at the community hospitals. The program offers comprehensive nephrology services including the management of acute kidney injury, chronic kidney disease, hypertension, glomerulonephritis, acute dialysis, chronic dialysis, home dialysis therapies, continuous renal replacement therapy, and renal transplantation. The successful candidate should have strong communication skills and be experienced in coordinating patient care with hospitalists, radiologists, pathologists, and other subspecialists.

Required Licensure/Certifications
