Job summary

Carease Health seeks a Travel Nurse Practitioner to provide national clinical coverage in skilled nursing and post-acute settings. This role combines travel with remote APCM support when not traveling.

You will perform patient assessments, develop care plans, and collaborate with facility teams, while maintaining compliant documentation and navigating multi-state licensure and credentialing requirements. Extensive travel and licensing support are provided.

Qualifications

• Active Nurse Practitioner license in good standing.
• National board certification as NP.
• DEA registration (or ability to obtain).
• Ability to obtain licensure/credentialing in multiple states.
• Strong clinical assessment, decision-making, and documentation skills.

Responsibilities

• Provide in-person patient care within SNF and post-acute settings nationwide.
• Deliver temporary coverage for open territories and absences.
• Perform comprehensive patient assessments and treatment planning.
• Maintain timely, accurate clinical documentation.
• Collaborate with facility leadership, nursing staff, and Carease Health teams.
• Support continuity of care during transitions and onboarding.
