Job summary

Community Health and Wellness, LLC seeks an Independent Contract Music Therapist to serve children and youth with disabilities in Texas, traveling to their homes to provide focused music therapy sessions.

You will document each session, set tailored goals, communicate with the care team, and ensure timely reporting, invoicing, and HIPAA-compliant note-keeping using Word, Excel, and Adobe Reader. An MT-BC credential and private laptop are required.

Qualifications

• Must travel to current or prospective clients’ homes.
• Must communicate with health care professionals, clients, families, and the public verbally and in writing.
• Must have access to a private laptop or desktop for HIPAA compliance.
• Track SMART goals and progress using Word, Excel, and Adobe Reader.
• Implement patient care plans through home and community activities.
• Work independently and manage time with clients.

We are seeking an Independent Contract Music Therapist to serve for the following:

• Provide one-on-one Music Therapy services to children and youth with congenital disorders/intellectual disabilities/physical disabilities


• Create tailored goals to meet the needs of individual clients


• Document on each session


• Communicate with other team members and service providers


• Turn in timely documentation including weekly invoicing, progress notes, and related paperwork

Essential Duties and Responsibilities

• Must be able to travel to current or prospective clients’ homes


• Must be able to communicate with health care professionals, clients, families, and the general public through verbal and written communication


• Must have access to a private personal laptop or desktop computer for HIPAA compliance


• Track SMART goals and progress through weekly documentation using Microsoft Word, Excel, and Adobe Reader


• Implement patient care plans through home and community goal-related activities


• Able to work independently, manage one’s own time and the time of client(s)
