Qualifications

• OT license and state certification required, CPR/First Aid current.
• Experience providing on-site OT assessments and individualized treatment plans.
• Document progress notes and maintain compliant client records.
• Adhere to ethics, privacy laws, and workplace safety guidelines.
• Set measurable goals aligned with patient needs and program targets.
• Collaborate with multidisciplinary team to optimize outcomes.
• Pipeline-driven caseload planning and progress tracking.
• Educate patients and caregivers on therapy plans, home exercises and safety.
• Participate in performance metrics reporting and productivity tracking.
• Maintain ongoing professional development and Certifications.
