Job summary

WesleyLife in Johnston, IA is seeking an experienced MDS Coordinator to oversee comprehensive MDS assessments, CAAs, and care plan reviews in line with CMS RAI guidelines.

You will manage MDS schedules, ARDs, Medicare/Medicaid reviews, ensure accurate documentation, and support CMS reporting while traveling to assigned communities to provide onsite training and coverage as needed.

Qualifications

  • Graduate of an accredited nursing program.
  • Current Iowa RN or LPN license in good standing.
  • 2 years of long-term care experience.
  • 2 years of dedicated MDS experience.
  • RAC-CT certification required within one year of hire.
  • Strong knowledge of CMS RAI guidelines, Medicare, Medicaid, and long-term care regulations.
  • Proficiency with Microsoft Outlook, Word, and Excel.
  • Excellent organizational, communication, and time-management skills.
  • Ability and willingness to travel regularly.

Responsibilities

  • Complete, review, and submit MDS assessments and care plans per CMS RAI guidelines.
  • Manage MDS schedules, ARDs, Medicare assessments, IPA reviews, and Medicaid Case Mix processes.
  • Ensure accurate clinical documentation, ICD coding, and regulatory reporting.
  • Monitor MDS quality measures, validation reports, and compliance metrics; complete corrections as needed.
  • Educate and support nursing and interdisciplinary team members on MDS best practices.
  • Participate in Medicare meetings, QAPI initiatives, and quality improvement activities.
  • Identify changes in resident condition and coordinate appropriate assessments.
  • Travel to assigned communities to provide onsite support, training and coverage as needed.

Key Responsibilities
